Visiting Angels is a compassionate and patient-centered home health care provider, dedicated to delivering high-quality services to individuals in the comfort of their homes. We are seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team on a part-time basis. This role will focus on intake assessments, follow-up assessments, and skilled nursing care for our home health patients as well as maintaining patient files.

Position Overview

The ideal candidate will be responsible for conducting initial patient and follow-up assessments, developing care plans, and performing follow-up evaluations to ensure the highest quality of care. This position requires strong clinical skills, attention to detail, and a passion for patient-centered care.

Key Responsibilities

Conduct initial intake assessments to evaluate patients' medical history, home environment, and care needs

Develop and update individualized care plans based on assessment findings

Perform follow-up assessments to monitor patient progress and adjust care plans as needed

Provide skilled nursing care, including medication administration, wound care, and disease management

Educate patients and families on self-care, medication management, and home safety

Collaborate with physicians, therapists, and other healthcare providers to ensure continuity of care

Maintain accurate, timely, and compliant documentation of all patient interactions

Requirements

Current LPN or RN license in [State]

Minimum 1 year of nursing experience (home health experience preferred)

Experience in conducting patient assessments and developing care plans

Strong communication and clinical decision-making skills

Ability to work independently and manage time effectively

Valid driver’s license and reliable transportation

Proficiency with electronic medical records (EMR) systems preferred
