ahead as you do your life's best work.(sm)Somerset Internal Medicine has an immediate full-time openingfor a friendly, patient focused and detailed oriented RN to join our team. This is a full-time position (40 hours perweek) 8:30 am -5:00 pm. The RN hiredinto this position might be expected to be flexible with their schedule andwill be cross trained with rooming patients.May be asked to float to other practices for coverage. Schedule to be determined upon hire.Primary Responsibilities:

Order vaccines as needed

Oversight of the medical teams CLIA competences

Serveas liaison between patients, staff and providers

Supportthe care team and its patients by providing and documenting nursing carefollowing established standards and practices

Provide patient care and educationand assist in locating and coordinating services from other providers andresources

Serve as liaison between patients,staff and providers

Document all patient interactionsand other clinical activities accurately and in a timely manner in theelectronic health record (EHR).

Demonstrate respect for patients andProHealth staff by presenting a friendly, courteous manner at all times andmaintaining patient confidentiality

Provide attention to patients in amanner that builds confidence, trust and loyalty.

Act as a member of the care team,including preparing patients to be seen by collecting and recording (asapplicable): Height and weight, Temperature, Blood pressure, Headcircumference, Current medications and Chief complaint and history

Triage patients according toapproved protocols and consistent with ProHealth’s Appointment and SchedulingGuidelines. Answer and document patient phone calls; take complete messages androute to providers as needed using the EHR

Provide and document patient caresuch as giving injections and performing preventive screenings

Educate patients regarding theirillness or injury and provide instruction regarding self-management of theircondition

Carry out standing orders (i.e.,authorizing prescription refills and ordering tests) consistent with ProHealth’spolicies and procedures

Schedule tests, treatments andfollow up visits on behalf of patients. Assist patients with identifyingsuitable outside resources for disease management and other services; referpatients when appropriate

Monitor task list in order torespond to tasks in a timely manner, follow up on overdue orders and monitorstatus of critical referrals

Order supplies and vaccines andmaintain appropriate records. Prepare and maintain equipment includingsterilization

Maintain knowledge of currentcoding, OSHA and CLIA regulations and ProHealth policies

Assist care team to assure smoothoffice operation and delivery of excellent service through teamwork

As appropriate, conduct outreach topatients in need of appointments or other services, to communicate test resultsand other information, and to follow up after appointments to be sure patientsare following their plan of care

Professional education for nursingand Connecticut RN licensure

Basic computer knowledge andaptitude

Experiencein Acute or Primary care setting

Phone Triage

Employeesare required to screen for symptoms using the ProtectWell mobile app,Interactive VoiceResponse (i.e.,entering your symptoms via phone system) or a similar UnitedHealth Groupapproved symptom screener prior to entering the work site each day, inorder to keep our work sites safe.Employees must comply with any state and local masking orders. Inaddition, when in a UnitedHealth Group building, employees are expected towear a mask in areas where physical distancing cannot be attained

You will be provisioned with appropriate Personal ProtectiveEquipment (PPE) and are required to perform this role with patients and memberson site, as this is an essential function of this role
