Position Profile Bloom Hospice is seeking a dedicated and compassionate Team Lead On-Call RN to oversee and coordinate after-hours hospice care for our patients and families. As a Team Lead On-Call RN, you will be the primary point of contact during after-hours, ensuring that our patients receive high-quality, comprehensive care. You will collaborate closely with our interdisciplinary hospice team to address the physical, emotional, and spiritual needs of our patients. Additionally, you will provide education to patients and families about the disease process, manage and control symptoms, and offer support and strength during challenging times. Care delivery primarily occurs in the patient’s home or within a community setting. For Weekend Person: This role will work Friday to Sunday with shifts in schedule as needed. This role will average a 60/40 split on performing visits during their shift/administrative, support items, and troubleshooting. Responsibilities:

Oversee After-Hours Care: Provide leadership and support to teammates and provide direction for hospice patient care during after-hours, addressing immediate needs and coordinating with the hospice team as necessary.

Symptom Management: Conduct effective pain and symptom assessments, developing and implementing management plans.

Patient and Family Education: Deliver comprehensive teaching to patients, families, and caregivers about the disease process, symptom management, and end-of-life care.

Ethical Decision-Making: Navigate end-of-life ethical issues with sensitivity and professionalism.

Documentation: Ensure timely and accurate documentation of all patient interactions and care provided.

Resource Management: Utilize resources effectively and efficiently, optimizing care delivery.

Continuous Improvement: Adhere to the scope of practice, continuously seeking to improve processes and services.

Professional Conduct: Uphold high standards of personal and professional conduct at all times. Report issues and concerns to the Clinical Supervisor

Proactive Customer Service: Working with After Hours teams, Partners, Families and Caregivers to respond timely to any Grievances. Handoff to Clinical Supervisor for final resolutions.

Qualifications:

Educational Background: Graduate of an accredited school of nursing.

Licensure: Current CO RN licensure in good standing.

Experience: Minimum of one year of recent experience in medical, surgical, or critical care as a professional nurse.

Clinical Skills: Strong clinical assessment skills.

Communication Skills: Excellent communication and interpersonal skills.

Problem-Solving: Solution-driven, creative, and resourceful problem-solving abilities.

Technical Proficiency: Proficient computer skills.

Specialized Experience: Previous hospice or oncology experience.

Knowledge: Understanding of the death/dying and bereavement process.

Certification: CHPN (Certified Hospice and Palliative Nurse) certification preferred but not required.

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $100,000 - $110,000 annually with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
