Speech Pathologist Boulder Community Health, Boulder, CO 80303 $39.56 - $59.18 an hour - Per Diem, Days. Includes Per Diem Differential. +$2.50 for weekend shifts. Boulder Community Health is seeking a Speech Pathologist who assumes responsibility and accountability for providing quality care to patients with disorders affecting language, speech, cognition and/or swallowing functions. Speech/language therapy services include the following: assessment of patient level of function/needs assessment, development of treatment programs, provision of treatment and education of the patient, family and caregivers as related to language, speech, cognition and/or swallowing functions. One year of intensive exposure to speech-language pathology within the hospital environment with primary emphasis on adult acquired neurogenic disorders of communication/cognition and swallowing is preferred Experience in an outpatient rehabilitation environment with an interdisciplinary team is preferred.Responsibilities Practices in a safe manner that minimizes risk to patients, self and others Provides speech-language pathology services including evaluation, test interpretation, treatment planning, therapeutic interventions, education, and discharge planning in accordance with all departmental and professional guidelines Establishes an individualized plan of care and functional goals, involving the patient/family in the goal setting process, making the patient a partner in the rehabilitation process Assesses patient’s response to treatment and revises plan of care as indicated Accurately completes documentation in a timely manner in accordance with all departmental and professional guidelines Maintains complete, timely, and accurate billing practices Promotes effective communication and teamwork within Outpatient Rehabilitation, the Boulder Community Health system, and the community Participates in the establishment of department goals and in the achievement of department and BCH goals Complies with all regulatory requirements, including Joint Commission Standards Is familiar with organization, department and job specific Environment of Care areas, including Life Safety, Utilities Management, Hazardous Materials Communications, Emergency Preparedness, Infection Control and Medical Equipment FailureAbout Boulder Community Health and Boulder, Colorado Boulder Community Health’s vision is partnering to create and care for the healthiest community in the nation.
