Job Responsibilities:
Conduct regular visits to patients’ homes to deliver high-quality care.
Perform patient assessments and monitor vital signs to ensure appropriate care.
Administer medications, provide wound care, and assist with daily living activities per visit.
Visits are one to two times per week depending on physicians order.
Educate patients and their families on proper home health care strategies and procedures.
Collaborate with other healthcare professionals to plan, implement, and evaluate patient care plans.
Document and maintain accurate patient records.
Qualifications:
Valid LVN or RN license.
Strong interpersonal skills with the ability to communicate effectively with patients, families, and team members.
Reliable transportation and willingness to travel within the community to visit patients.
