We are currently looking to add a new member to ourdynamic team for Case Manager - TAY Achievers.This position is responsible for coordinating Transitional Age Youth (TAY) financial assistance needs with the funding sources available.The TAY Achievers Case Manager will help empower TAY to achieve Self-sufficiency and to establish essential community connections to meet client’s needs. The TAY Achievers Case Manager will also carry a caseload to support TAY Achievers eligible youth.

COMPENSATION

-Starting hourlyrate:$24.15

-Full Time, Non-Exempt classification per FLSA

-Spanish Bilingual differential -10% increase on the employee's hourly base rate of pay

-Mileage reimbursement $0.67 cents per mile

-403(b) plan which allows employees to contribute a portion of their salary pre-tax toward their retirement with a monthly employer match. Eligibility provisions apply.

-13paid holidays annually

-17 days of PTO accrued annually (increases after 3 years of service)

QUALIFICATIONS

-A Bachelor's Degree from an accredited college or university in a related field. Preferred degrees include psychology, social services, social work, counseling, human/child development, education or a related field of study with experience in assisting persons in independent living skills in a human services-oriented public/private organization; or

-An Associate's Degree from an accredited college or university in human services (psychology, social services, social work, counseling, education, human/child development) or

-A related field of study, and 2 years of experience in assisting persons in independent living skills in a human services-oriented public/private organization.

-Knowledge of social work practices, case work methods, theories of behavior and behavior management, theories of human development and psychology.

-Be of good character and reputation, professional, communicate effectively, and able to follow agency policy & procedures

-Be flexible in schedule availability, which can include split shift,evenings,weekends and holidays

-Must be available to workonsite atFCNI offices and/orclient locations

BASIC DUTIES

-The TAY Achievers Case Manager will provide support to the Transitional Housing Placement Program, including support to THPP-M, THPP-NMD, & THP+, in addition to all ILP eligible TAY with needs related to enrolling in or maintaining enrollment in a College, University, or Vocational Program that are not covered by Financial Aid, Student Loans, Chafee Grants, etc.

-The TAY Achievers Case Manager must enhance TAY’s ability to achieve independence and self- sufficiency. Examples of TAY Financial Assistance Program Support include:

Housing in a dormitory, student housing or apartment

Transportation needs

School supplies, textbooks, lab fees, or other required school-related tools

Meeting urgent, essential, or emergency needs related to living, e.g., utilities, household items, furnishings, etc.

-The TAY Achievers Case Manager will train FCNI, DSS, Probation and ILP staff to encourage planning and exploring other resource options in order to minimize crisis or emergency requests.

-Staff with direct services training may be asked to provide direct services when agency needs require themto do so, including during evenings and weekends.

-The TAY Achievers Case Manager must track outcomes and data related to the program.

-Other duties as assigned
