Night Care Coordinator

The Evan Jacobson Special Needs Trust is seeking a professional who is dedicated to assisting individuals with disabilities improve their quality of life.

We are currently recruiting applicants for a Care Coordinator position in a hybrid ABA/caregiving program for Evan Jacobson, a young adult man with severe epilepsy and autism. Evan receives care from PRAISE Behavioral Services during the day and from IHSS (In Home Supportive Services) overnight.

As the Care Coordinator, you will be responsible for managing the IHSS portion of Evan’s care, including hiring and leading IHSS caregivers. (As Care Coordinator you can, optionally, take on an active role as an IHSS caregiver yourself – this is highly encouraged.)

As Care Coordinator, you will oversee the smooth operation of overnight shifts. You will ensure that qualified caregivers are available every night. You will be the point of contact for handling emergencies during the IHSS portion of Evan’s care.

Responsibilities include, but are not limited to:

You will find and hire qualified night caregivers

- Understand requirements for caregivers

-Advertise for, interview and recruit potential caregivers

Some of these caregivers may be PRAISE staff, working extra hours separate from the PRAISE program

- Ensure that caregivers have correct qualifications and are a good fit for working with Evan

- Guide caregivers through the process of registering as a Provider through IHSS

- Hire caregivers through IHSS website

You will work with PRAISE Behavioral Services on an integrated PRAISE/IHSS Caregiver Schedule

- PRAISE is responsible for the day program and overall schedule

-PRAISE is responsible for identifying the time slots which need coverage by IHSS caregivers

- As Care Coordinator, you are responsible for scheduling IHSS caregivers, communicating the schedule to the caregivers and ensuring that they cover the timeslots

You will be the escalation path for reschedules & no-shows

- If an IHSS provider caregiver reschedules or doesn’t show, you will be responsible for getting another caregiver to take their slot

- In no other caregivers are available, you will cover that time slot yourself

- If you become an IHSS hired provider, these would be paid hours via IHSS

You will have emergency oversight over IHSS providers

- They will contact you for guidance if Evan has a seizure or if there is another emergency

-You will review responses to check that they were correct per guidelines

- You will review IHSS provider Time Sheets

Ensure that time sheets are turned in on time

Become an Authorized Representative for Timesheet Approval via IHSS

Approve timesheets via IHSS website

You will be responsible for coordination with IHSS, including

- Monitoring progress of caregiver applications with IHSS

- Working with IHSS to resolve timesheet issues (common with new providers)

- Compile the annual IHSS Assessment Package

IHSS Health Assessment Form

Health Assessment (in coordination with PRAISE)

IHSS Behavioral Assessment form (from PRAISE)

Be present for yearly IHSS In-home Assessment

Requirements (also including requirements for IHSS caregiver) :

Have a telephone and reliable method to record scheduled appointments and return phone calls within 24 hours.

Have reliable transportation, possess a current CA driver's license, and current liability insurance for the vehicle.

Computer literate; able to communicate with IHSS, PRAISE, Evan Jacobson Special Needs Trust, etc. via email, WhatsAp. Use IHSS and other websites.

Current TB clearance

DOJ/FBI clearance

1-2 years of experience is working with learners with disabilities, caregiving, teaching, or other related experience.

Stand, sit, twist, kneel, bend-over, crouch down, walk, and sit/play on the floor for extended periods of time.

Be able to lift, push and pull up to 40 pounds without assistance from another adult.

Must be self-motivated, responsible, honest, articulate and able to work independently with minimal supervision.

Present a professional demeanor, be well organized, and pay attention to detail.

Have excellent time management skills.

Be comfortable working in homes (studio apt., housing projects, etc.) of families with limited resources.

Maintain professional boundaries, professional composure, and work well under pressure.

Politely interact with individuals and families who may be uncooperative or aloof.

Maintain professionalism if and when a client aggresses, self-injures, tantrums, screams, etc.

Effectively communicate with others (clients, family members, agency staff)

Be comfortable around adults with autism

Is confident, calm and positive when working with challenging behaviors

Comfortable with video and audio recording in client’s home
