Community Choice Active Life Center provides day program services, positive behavior support and community integration assistance to adults with developmental disabilities.

Position: Community Integration Facilitator

We currently have full-time and part-time positions.

Full-time: Monday-Friday 7am-3pm or 9am-3pm

Part-time: Monday-Friday 9am-1pm or 10am-2pm

Starting Pay: $18.50- $20.50 per hour depending on experience.

As a Community Integration Facilitator your responsibilities would be facilitating community inclusion.

The 9am- 3pm and part-time shifts would start at the day program facility.

Your responsibilities would be providing direct support and supervision which would also include transporting clients out in the community in a company vehicle to activities such as exploring the central coast, going to local parks, beaches, shopping, out to eat, bowling, golf range, library, art studios, walks in the community etc.

You will also be required to facilitate an environment were clients can work on personal goals. You will be responsible for providing documentation of goals.

The 7am-3pm shift would include:

Assisting clients Monday-Friday from 7am-9am in their home and 9am-3pm in the community.

Between 7am-9am your responsibilities would involve assisting clients with personal care, hygiene and breakfast.

At 9am prepare to transport clients in a company vehicle to day program for morning movement time, activities and snack before preparing for community integration activities. Your responsibilities would include providing direct support and supervision and maintaining a safe environment at all times in the home and in the community. While in the community you will provide transportation using a company van and transport clients to and from activities within the surrounding area and facilitate a learning environment were clients can work on personal goals. You will be responsible for documenting goals.

Qualifications:

High School diploma or GED required.

Must be 18 or older.

Have a valid California D.L

Clean driving record with two years of verifiable driving experience.

Pass background check, physical ability exam and negative TB test.
