The Clinical Social Worker/therapist is part of a multidisciplinary team that includes medical providers, nurses and case managers. This position will be mobile and integrated into SFCHC’s HIV Services Department. This position will function as a clinical leader on the team, working to engage clients with acute mental health and medical challenges into street-based medical care. Clients served will include the LGBTQ+ community, people who are experiencing homelessness, people living with HIV, active substance users and people struggling with unmanaged mental illness. The social worker/therapist will carry a caseload of therapy patients and will provide individualized social work services to clients so that they can achieve more of their personal and wellness goals.
Essential Functions:
- Conduct field-based intakes, assessments, and reassessments of clients with mental health and substance use disorders.
- Provide consultation to the interdisciplinary team for urgent situations and assist with treatment planning and development.
- Carry a caseload of field-based therapy clients while being able to provide brief behavioral health interventions.
- Conduct field-based crisis intervention with the ability to assess for risk, a psychiatric emergency and conduct a 5150 assessment.
- Be a liaison between all psychosocial services and the client’s medical provider to ensure that a client’s care is coordinated.
- Develop and utilize multiple modalities to address clients’ substance use and mental health needs, including but not limited to individual psychotherapy using evidence-based treatments.
- Continually update client care plans and develop appropriate discharge plans while coordinating with outside parties including hospital social work teams, psychiatric emergency providers and urgent care, etc.
- Lead and participate in all required meetings, including huddles, case conferences, monthly staff meetings and in-service trainings.
- Provide service referrals, linkages, care coordination, and advocacy services for clients, and monitor the outcomes of these activities.
- Help care teams tailor health education for the client and provide support for medication adherence.
- Provide Clinical Supervision to case managers as needed.
- Complete necessary documentation, assessments, and treatment plans in a timely manner.
- Perform other duties and tasks as assigned.
Minimum Qualifications:
- Master’s degree in Social Work or equivalent
- ASW or AMFT registration is required. License is preferred
- One year experience of clinical expertise in assessing and managing the psychosocial needs of low-income populations, multiply diagnosed patients (HIV, mental disorders, and substance use), LGBTQI folks, immigrants and refugees
- One year experience with harm reduction, client-centered counseling, motivational interviewing, and other evidence-based treatment approaches
- One year experience working in interdisciplinary teams. Must be able to work independently and have an adaptable working style and be a strong contributor to team performance
- Demonstrated reliability and good interpersonal, organizational, verbal, written communication skills. Must be able to handle multiple tasks and have ability to work professionally and ethically within multi-racial and multicultural settings
- Be able to provide customer service in an effective, courteous, and friendly manner while meeting the needs of diverse clientele
- Knowledge of legal mandates and ethical issues related to client privacy and confidentiality
- When eligible, completion of an Involuntary Hold/5150 certification
- Must be able to sit, stand, and walk for extended periods of time
- Fully vaccinated against COVID-19, complete with booster shot(s) and able to show proof of vaccination
- Up to date with vaccinations, especially seasonal flu shot vaccine
- Proof of TB clearance (issued within the past 12 months or less)
