JOB SUMMARYThe spirit of Life Steps’ culture is client-individual focused, with the pursuit of quality services and a high level of performance. The primary responsibility for the Lead Quality Management is to aid the Management team in ensuring in-home quality control to individual(s), regional center, and staff satisfaction. The Lead Quality Management will work collaboratively with both the Program Supervisor and Program Manager to assist in training/re-training of employees properly, completion of in-home quality control paperwork and adhering to Life Steps’ performance standards and policy. Additionally, the Quality Management will aid the Program Supervisor, to ensure employees perform according to the ISP.Ensure that individuals receive the highest quality services by implementing ongoing quality assurance measures:Work with Supervisor to conduct quality management reports regarding individuals’ safety, staff safety and the services we provide.Provide recommendations for program improvements and implement interim and long-term solutions concerning individuals and staff.Facilitate and organize quality improvement initiatives and activities.Work in coordination with the TCQM and Human Resource Department on potential investigations concerning staff and individuals.Execute initiatives per the request of the Manager and/or Supervisor.Meet with Supervisor weekly, to discuss cases, any issues raised by individual or staff, set action items, evaluate performance, and other items by agenda.Ensure excellent service is provided to the individuals we serve by conducting planned and random site visits.Assist Supervisor, if necessary, to track reports, review/complete ISP reports, and ensure delivery to Regional Center.Assist Supervisor in training front-line program staff to ensure they have the skills necessary to deliver quality service to individuals.Conduct weekly problem-solving dialog with Supervisor and LCSF/CSF/DSP regarding individuals, if needed.Provide resource information to Supervisor or staff on an ongoing basis relating to individual issues.Assist Supervisor and Manager in providing training, retraining and ongoing training of LCSF/CSF/DSP that will benefit the individual.Assist Supervisor, in creating and maintaining LCSF/CSF/DSS staffing and formulate schedules.Attend meetings with the individual, Regional Center and staff to assure quality services.Assist Supervisor by working with staff on issues that arise in the course of working with individuals, Regional Centers or other third parties.Assist Supervisor in any way they deem necessary for insuring the health and safety of the individual.Participate in Life Steps Foundation annual training plan.Monitor Direct Support Professionals to ensure that they meet Life Steps’ high standards for performance.Observe Direct Support Professional for refresher training and provide recommendation to Supervisor and/or Manager. Refresher trainings can take place during monthly supervision meetings or in-home.Assist Supervisor in reviewing and/or complete Special Incident Reports and Adult Protective Services Reports with the associated Trending Forms to match.Assist Supervisor in Direct Support Professional(s) performance evaluations.Consult with Supervisor and implement performance improvement procedures where necessary to enforce Life Steps policies and procedures.Emergency ResponseMandatory participation in a rotation of back-up emergency on-call.Immediate response to the primary emergency assistance phone as holder of the back-up emergency phone, as assigned.Assist with training and retraining of LCSFs in responding to the primary emergency assistance phone.Assist with training and retraining direct support staff and individuals on how to utilize the emergency response system.Maintain professional and technical knowledge.Attend 15 hours of in-service and continuing education training per year directly related to services for developmentally disabled adults.Attend meetings and conferences as appropriate to further develop and improve performance and clinical effectiveness.Maintain awareness of development of best practices current in the field, provision of social services, and other job-related areas.Maintain high standards of personal conduct in the capacity of a Quality Care and Training Facilitator.Maintain professional boundaries with individuals receiving services, their family members and other support service persons at all times.Respect the privacy of the individuals receiving services, their family members and other support service persons and maintain confidentiality of personal health information in accordance with HIPAA regulations.ESSENTIAL JOB QUALIFICATIONSBA in psychology, human development or social service or 3 equivalent years’ experience or training in direct service to consumers with developmental disabilities.Good verbal and written communication skills.Strong team building skills. Proven leadership and supervisory abilities.Must be able to get along with co-workers, supervisors, employees at governmental agencies, individuals and the individuals’ families.Must be able to lift/transport consumer(s) in event of emergency.Current certifications in CPR and First Aid.Must have valid California Driver’s License and current automobile insurance.Must be able to attend monthly staff trainings and supervision.Basic computer skills and word processing necessary.PHYSICALWhile performing the duties of this job, the employee is regularly required to stand; walk; sit; use hands to finger, handle, or feel; reach with hands and arms; stoop, kneel, crouch, or crawl and talk or hear. Frequent walking, including occasionally walking upstairs and downstairs. Performs some repetitive motion activities. The employee is occasionally required to climb or balance. The employee must regularly lift and /or move up to 20-25 pounds.
