The Position
The Senior Wellness Navigator (part of the Prevention and Early Intervention Program) is responsible for working collaboratively with older adults living in identified independent living properties throughout Santa Barbara county (such as subsidized apartments for seniors).
The Senior Wellness Navigator will help older adults increase access to community resources and will offer the opportunity to engage in enriching and community-building activities for seniors living in identified independent living properties to reduce isolation and loneliness. The Senior Wellness Navigator will identify risks, promote strengths, and equip older adults and caregivers with the skills and knowledge needed to overcome challenges and stabilize. The Senior Wellness Navigator will utilize targeted case management and connection to resources for seniors living in identified independent living properties and incorporate advocacy efforts. In addition to case management, this position includes planning, organizing, and scheduling engaging and stimulating activities for the residents, and promoting these activities to enhance the lives of the older adult residents. Services will be provided at designated independent living senior properties throughout the county of Santa Barbara, as well as other non-affiliated properties. This position reports to the Senior Wellness Promotion Program Supervisor.
This position can be based out of any of FSA’s offices in Santa Maria, Lompoc, or Santa Barbara. Some travel within SB County may be required. Sample Job Duties and Responsibilities
1. Work collaboratively with older adults in senior independent living settings to promote engagement in site-based activities and provide services, including case management, advocacy, and linkages to community resources, with the goal to equip older adults with the skills and knowledge needed to overcome challenges.
2. Meet with residents on site via weekly office hours for appointments and drop-ins.
3. Plan, promote, schedule, and conduct programs and activities tailored to the needs and interests of the housing property residents.
4. Evaluate activities to determine appropriateness and interest level of participants, and make changes as needed.
5. Coordinate logistics of activities including equipment, decorations, entertainment, refreshments, etc., in coordination with other team members.
6. Create a monthly activities calendar including scheduling speakers and coordinated activities. Creation and distribution of associated marketing materials for residents as needed.
7. Actively encourage resident participation in the activities, scheduled speakers and engagement activities. Document resident attendance at activities.
8. Collaborate with on-site property managers and other representatives affiliated with senior properties to ensure successful service delivery.
9. Utilize assessment skills and screening tools to identify risks and strengths, and determine assistance needed. Utilize CSQ-8 satisfaction survey to measure satisfaction with the services, and a second evidence-based assessment tool (pre-post) will be identified to measure life satisfaction. In addition, some of the activities conducted may be evidence-based, such as a mindfulness wellness group.
10. Exhibit a working knowledge of community resources and provide appropriate referrals/linkages to support access to social, educational, and health services.
11. Connection to and support navigating various community resources.
12. Assist clients with reading and understanding documents.
13. Provide assistance in Advanced Care Directives and end of life planning.
14. Build Tools for successfully aging in place.
Employment Requirements:
1. Bachelor’s degree in social work, gerontology, psychology, sociology required; two years of equivalent, related experience accepted in lieu of BA.
2. Bilingual in English and Spanish required.
3. Exhibit cultural competency.
4. A minimum of one year work experience in a community setting. Experience working with at-risk seniors desirable.
5. An ability to relate warmly and professionally to older adults and community organizations.
6. Computer and Internet literacy.
7. Background check, TB, and drug screen clearance required.
8. Valid automobile insurance and a valid California Driver License are required.
9. Reliable transportation is necessary to travel throughout Santa Barbara County.
10. Physical tasks, such as moving tables/chairs, lifting boxes up to 15 lbs., etc. will be required.
Classification, Hours, and Pay Rate:
This position is full-time (40 hours/week), non-exempt, with including: medical, dental, vision, life insurance, flexible spending accounts and 403(b) retirement plan. Generous vacation, holiday, and sick leave also offered.

Pay range is $21.00-$25.00/hour.
