Case Manager I (TCCM) Our Transitional Care and Case Management (TCCM) department is seeking a Case Manager I to provide coordination and support to clients/family in their transition from nursing facilities/long term residential facilities with the goal of coming back into their community. A Case Manager will support their client with securing affordable housing, navigating through Government benefits and helping locate adequate services like In Home Support Services (IHSS) or even nursing support. Compensation: $26.69 per hour Schedule: A Case Manager works a full-time schedule, Monday through Friday typically between the hours of 9:00AM-5:30PM . Special Skills and Knowledge: Understanding and commitment to the philosophy of inclusion, social equity, advocacy, and consumer-driven services. Ability to anticipate and initiate action, problem solve, and make recommendations. Advanced organizational skills. Effective written and verbal communication skills. Ability to make independent judgments and work with minimal supervision. Strong time management and ability to be extremely flexible with scheduling. Qualifications:

Two years of experience working in the field of human services; experience working with programs such as IHSS and Medi-Cal preferred.

Bachelor’s degree (education can be substituted with additional 2 years of experience in the field).

Possession of valid California driver’s license with a driving record that meets agency standards. Auto insurance and access to a vehicle.

Duties and Responsibilities:

Help clients obtain housing readiness documents (i.e ID, Social Security Card, birth certificate, and income verification)

Research new housing opportunities and apply clients to all housing opportunities available to them

Maintain good relationships with landlords/housing agencies/housing authority

Help clients attend housing interviews/meetings

Assist clients to schedule and travel to all medical and dental appointments

Money Management (e.g. banking and budgeting)

Ensure that your client has access to free or low cost transportation in the community

Connect your client to community programs and benefits

Act as liaison between EBI and other State and Local agencies, i.e. Housing Authority and Social Security

Maintain accurate and detailed records, including State and program required documentation, case notes, billing information, and client file

Create a comprehensive service plan to ensure ongoing services in the community and support in transitioning of institutional settings to housing

The Public Service Loan Forgiveness program is a federal program that forgives student loan debt for nonprofit employees after 120 qualifying payments.
