VCC is an equal opportunity employer.ResponsibilitiesResponsible for assisting with coordinating and providing panel management and population health services to patients within the practice who are at the most risk for health deterioration or poor outcomes. Improve health outcomes through scheduling preventive and maintenance care appointments, educating patients, building the medical home between patients and practitioners and enhancing communication and continuity of care.Implement actions for increasing productivity such as, scheduling patients for PCP follow-up visits and specialty appointments, coordinating any needs prior to appointment (including translation, transportation, companionship, etc.), and anticipating open slots for same day appointments and proactively maintaining full provider schedulesMeet weekly with the Care Team to identify and implement actions for improving population management outcomesOrganize weekly pre-clinic team huddles to update on the progress of various goals—such as Meaningful Use, PCMH, compliance Coordinate monthly team meetings with clinic site Registered Nurse to discuss progress towards PCMH goals and areas needing improvementProvide end-of-visit interviews, coaching and patient engagement conversations when necessary and document in EHRReinforce information given to the patient and/or family with handouts to improve patient self-management skills and communicationQualificationsMinimumHigh school graduate or equivalent;Medical Assistant certificationMinimum two years’ back office Medical Assistant experienceBilingual English/SpanishValid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposesRequired Skills/Knowledge/AbilitiesAbility and willingness to self-motivate, prioritize, and be agreeable to change processes to improve effectivenessAbility to function at the highest level of licensing and competencyExcellent interpersonal skills; effective written and verbal communication skills Demonstrated ability to be culturally sensitive and appropriate in working with others and show respect for diversityAbility to present information effectively both verbally and in writing to individuals and groups from various cultural, ethnic, economic and educational backgroundsAbility to use critical thinking to make decisions and problem solvePay Range$27.00 - $30.00PI273491175 Apply
