Job Summary We are seeking a skilled Physical Therapist to join our team. The ideal candidate will provide physical therapy services to patients in various settings, focusing on improving mobility, relieving pain, and preventing or limiting permanent physical disabilities. Responsibilities - Conduct patient evaluations to assess their physical condition and needs - Develop individualized treatment plans based on patient assessment - Implement therapeutic exercises and activities to help patients improve movement, manage pain, and restore function - Educate patients and their families on at-home exercises and self-care techniques - Collaborate with healthcare professionals to coordinate patient care and progress - Document patient progress accurately and maintain up-to-date records Skills - Proficiency in home health care and orthopedics - Strong knowledge of anatomy and physiology - Experience working with geriatric populations - Familiarity with home community care systems - Expertise in physical therapy techniques and modalities - Ability to perform discharge planning effectively
