Ascencia is a nonprofit homeless services agency and emergency shelter located in Glendale, California, serving 1,200 people in need every year. Our work begins with a fundamental respect for the dignity of the people we serve, driven by the persistent and unfailing belief that no one should be without a home. Ascencia provides a range of services and housing to move homeless families and individuals in Glendale, Burbank, Northeast L.A., West Hollywood, East Hollywood, and the surrounding communities of Los Angeles off the streets and into their own home. These programs include street outreach, 45 beds of year-round emergency housing for families and single adults, comprehensive case management services, occupational therapy, on-site mental health care–including trauma therapy and psychiatry–and much more.

Description: The Outreach Case Manager is responsible for connecting to and engaging with homeless people on the streets of CD 4 in an effort to link them to services and housing. In addition to visiting commonly known places where homeless people congregate, the Outreach Case Manager responds to requests for service made by first-responders (police, fire), business owners and residents who encounter people who are homeless. The Outreach Case Manager is based in CD 4, works collaboratively with Case Management staff and when applicable, the Ascencia CES Access Center staff to ensure client needs are met.

Responsibilities:

Develop professional and supportive relationships with homeless persons staying in outdoor locations or other places not meant for human habitation in the greater Glendale area.

Identify and document client needs through repeated contacts with homeless persons;

Provide information regarding services and transportation as needed to people seeking services;

Respond to calls for assistance from first responders, business owners, residents and hospital discharge planners;

Work in tandem with the primary (Access Center) case manager to assist clients in reaching their case plan goals;

Continually monitor and evaluate each client’s progression through their case management plan, and develop corrective action revisions to the plan as needed;

Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and Ascencia guidelines;

Actively participate in client- and program-related meetings and trainings;

Attend program-related community, coalition and committee meetings;

Inform community and faith organizations about the work of the Outreach Team and how they can refer homeless people for services;

Maintain related administrative record keeping and reporting systems;

Prepare case management related reports including but not limited to: outcomes, successes, etc;

Adhere to strict boundaries and professional ethics in the care of others;

Conducts crisis intervention as needed.

Qualifications:

4 year degree in human services, or comparable combination of education/work related experience required;

Experience in social service setting with working knowledge of case management systems and planning techniques;

Knowledge of outreach techniques, related resources and the Continuum of Care for homeless persons;

Experience working with homeless individuals and families strongly preferred;

Must be proficient in the following computer applicationsMicrosoft Word and Excel, and be able to learn how to use the required Homeless Management Information System (HMIS).

Must have own transportation to conduct job related travel and must be able to drive agency vehicles.

Must meet insurance carrier’s requirements for approval to drive.

Must possess a valid driver license;

Drug and TB test will be required before start of work.
