CASE MANAGER
We are seeking an Alcohol and Drug Counselor/Case Manager to join our team! You will be responsible for facilitating groups, counseling clients on services available for treatment, helping combat relapses, and building long-term life skills to maintain sobriety.
Responsibilities:
-Conduct groups and 1:1 counseling sessions with clients per program standards and document progress and participation.
-Obtain clinical information from the intake assessment, medical hx questionnaire, biopsychosocial assessment and use the information to develop and implement a collaborative treatment plan.
-Maintains thorough clinical records for clients, including all services provided as part of individualized episode.
-Prepare progress reports and letters for relevant parties, if applicable.
-Educate families and groups on substance abuse treatments and prevention programs.
-Facilitate referrals to other healthcare or counseling professionals.
-Assists the client in discharge planning, identifying needs, and linking client to services
-Ensures documentation adheres to all regulatory time-frames and requirements
Qualifications:
-2+ years experience in counseling with a focus on group therapy and clinical documentation.
-Experience in conflict resolution
-Compassionate and caring demeanor
-Ability to build rapport with clients
-Excellent written and verbal communication skills
-Certification with a state of California counselor certifying organization
Minimum requirements for position are:
-Physical, TB test, Drug Screen, able to present in a professional fashion orally and physically, maintain and uphold strong boundaries while having a therapeutic manner.
-If recovering from substance abuse, must have at least one year of continuous sobriety.
-Knowledge of 12 step base recovery a plus but not mandatory.
-Pay scale will be determined by experience and education
