ALL IN ONE HHA is looking for a LVN to provide services at city of PERRIS, HEMET, SAN JACINTO, RIVERSIDE, CORONA, NORCO, COLTON, RIALTO, REDLANDS, HIGHLAND, SAN BERNARDINO city of FONTANA & MORENO VALLEY. Join a dynamic team of health care providers focused on health, wellness and prevention for all our patients. Provide and direct care services to patients, including assessment, treatment, program planning and implementation Work in accordance with physicians’ orders and adhere to applicable principles and practices of profession, ALL IN ONE HHA policies/procedures, and state regulations.
Conduct patients’ screenings regularly to determine need for intervention and treatment Coordinate treatment plan with the patient, family and other team members
Develop appropriate home or community programming to support the patient’s continued recovery in his or her own environment.
RN AND LVN FIELD NURSE NEEDED IN RIVERSIDE OR SAN BERNARDINO COUNTY
LVN SHIFT CARE NEEDED FOR 8 HOURS SHIFT MONDAY TO SUNDAY - INLAND EMPIRE.
12 HOURS SHIFT 7 AM - 7 PM & 7PM - 7AM MONDAY - SUNDAY ( no experience needed)
Active applicable California State LVN AND RN license
Current CPR/BLS
Recent Physical and TB Screen or Chest X-ray, FLU SHOT
Reliable Transportation / Auto Insurance / Valid Driver's License
