OverviewImmediate Opening in Belmont, CADignity Health Medical Group - Sequoia is seeking an experienced, an experienced , energetic and team oriented Primary Care (IM or FM) Advanced Practice Provider (PA or NP) in Belmont, CA. We are a multi-specialty group that is a physician led and patient centered organization. We are affiliated with Sequoia Hospital and aligned with one of the largest nonprofit health systems in the nation and the largest hospital system in California.Practice Highlights:

On-Site onboarding with Supervisory Physician

Phone access to Supervisory Physician and Group's Primary Care Providers

Dedicated ancillary team for efficient workflow

Internal pharmacy department handling daily refills

Compensation/Benefits Include:

Compensation Range: $120,000 - $150,000/year

Sign on, productivity and quality bonuses

Work from home 1 day per week

Conduct comprehensive health assessments.

Diagnose and treat acute and chronic illnesses.

Order and interpret diagnostic tests.

Treatment Planning and Management:

Develop and implement patient care plans.

Prescribe medications and therapies.

Provide health promotion and disease prevention education.

Collaboration and Consultation:

Collaborate with other healthcare professionals.

Consult with physicians and specialists as needed.

Coordinate patient care across various settings.

Qualifications

Physician Assistant or Nurse Practitioner experience

A valid and unrestricted California medical license

Board eligible or board certified in specified area of medical specialty (ABMS or AOA)

Completion of an accredited post-graduate training program

Pay Range$57.69 - $72.11 /hourWe are an equal opportunity/affirmative action employer.
