Full Time Registered Nurses/Case Manager

QUALIFICATIONS:

1. Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.

2. Two (2) years’ nursing experience, required.

RESPONSIBILITIES:

1. Ongoing interdisciplinary assessment of the patient

2. Communication with all physicians involved in the plan of care and other healthcare practitioners, as appropriate, related to the current plan of care

3. Participation in the HHA sponsored in-service training in case conferences

4. Participates and is available for on-call duty, nights, weekends and holidays.

5. Development and evaluation of the plan of care in partnership with the patient, representative, if any, and caregivers

6. Initiates and provides services in accordance with the plan of care.

7. Makes the initial OASIS evaluation, constructs the plan of care, and submits the documentation within 24 hours of the scheduled visit

8. Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner.

9. Informs personnel of changes in the condition and needs of the patient.

10. Counsels the patient and family/significant others in meeting nursing and related needs.

11. Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.

12. Determines the amount and type of nursing needed by each individual patient.

13. Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.

14. Provides patient and caregiver education

WORKING ENVIRONMENT:

Works indoors in Agency office and patient homes and travels to/from patient homes.

Previous home health experience preferred but not required.
