Southeast Senior Services is looking for a motivated individual to serve as the Case Management Regional Coordinator. This position provides Case Management services to seniors in multiple Southeast communities, as well as supervises the Case Managers, Service Coordinator, and Counselor. Job Duties include performing intake functions, assessing clients, developing care plans, arranging for services, monitoring services, discharging clients, preparing and submitting grant reports/materials, developing grant proposals, training and evaluating other Case Management staff, developing and monitoring program budgets, and developing program policies as needed.

KNOWLEDGE, SKILLS, AND ABILITIES:

Must have skills in-group process, problem solving, decision-making, excellent communication skills both written and oral. Excellent organizational skills, ability to manage time effectively, able to prioritize workloads, able to work within time frames and meet specific deadlines, and experience with computer word processing software. Good interviewing and assessment techniques. Have the ability to set realistic goals for staff and self and to work in a team environment.

Must have knowledge of management principles; principles and techniques of supervision and staff training and to supervise, evaluate employee performance and give feedback.

Ability to establish strong relationships with agencies, problem solve, trouble shoot to maintain those relationships and have knowledge in the areas of social support systems, interdisciplinary fields and issues affecting seniors.

Must work effectively with older people of varied ethnic backgrounds, demonstrate patience and work effectively with families, professional caregivers and community service agencies. Be creative and versatile in working with people of varying abilities and disabilities. Work independently as well as cooperatively with other staff

QUALIFICATIONS:

Two years of supervisory experience required. Five years of social services experience is preferred. Previous experience in assessments, care plans, case file reviews, and working with the elderly is strongly desired. Background check required.

This position is Full Time, starting salary $58,500-62,400/year DOE.
