    Summary

    To provide Social Work services to the Veteran population enrolled with the Sheridan VA HealthCare System. Services include provision of high quality case management, clinical treatment, advocacy, and coordination of linkages to other appropriate VA or community service providers and/or agencies as needed by the client. This is accomplished in collaboration with other members of the interdisciplinary treatment team.

    Responsibilities

    Clinical:

    Position requires the ability to conduct psychosocial assessments with a wide variety of individuals from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds. Social worker is able to communicate effectively, both orally and in writing, with people from varied backgrounds.

    The social worker incorporates complex, multiple causation factors, in establishing differential diagnosis and treatment of Veteran patients, including making psychosocial assessments and psychiatric diagnoses. Demonstrates ability to assess the psychosocial functioning and needs of patients, formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills, and assistance needed.

    The social worker is responsible for developing a seamless discharge plan for patients. The social worker will work closely with VA and community-based outpatient treatment teams, programs, agencies, and other applicable resources in establishing and coordinating a referral process for follow up care as appropriate. Will possess and demonstrate ability to establish and maintain effective working relationships with clients, staff, and representatives of community agencies, and may function as a liaison to the same. Possesses basic knowledge of Veteran benefits and services relating to special programs, service-connected compensation, non-service connected pension, etc. Facilitates referrals based upon veterans needs and eligibility.

    The social worker is responsible to work with a wide variety of acute, chronic and high-risk patients, i.e. those with traumatic brain injury (TBI), Post-Traumatic Stress Disorder (PTSD), Spinal Cord Injury (SCI), chronically mentally ill, suicidal patients, as well as other chronic or acute medical and mentally ill individuals. The social worker will demonstrate the ability to work with patients and families who are experiencing basic emotional, medical, and social problems utilizing individual, group, and family counseling skills.

    Will possess knowledge of psychosocial treatment modalities and the demonstrate ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques.

    The social worker is responsible to provide individual, group, and family psychotherapy and advanced level case management interventions used in the treatment of Veterans with polytrauma injuries, SCI, TBI, visual impairment, PTSD, addictions, and other serious disorders.

    The social worker has general knowledge of medical and mental health diagnoses, disabilities, and treatment procedures to include acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and basic medical terminology.

    The social worker develops and implements methods for measuring effectiveness of their individual social work practice and is able to demonstrate the ability to implement changes based on empirical findings. The social worker may serve on committees, councils, workgroups, and/or task forces and is a critical member of interdisciplinary teams.

    Administrative:

    The social worker will enter all veteran/family contacts in the electronic record using appropriate formats and templates. This information will be entered in a complete, confidential, and professional manner to insure information on the patient is shared with other VA staff. This information will be reviewed on a regular basis. The social worker will establish and maintain positive working relationships with employees, volunteers, consumers and stakeholders within the VA and outside community agencies. Social Worker will attend all appropriate staff meetings including but not limited to (interdisciplinary treatment team meetings, Social Work staff meetings, and perform other duties as required. The social worker will collaborate in the performance improvement processes and comply with performance measures as required by the VA for their specific population and/or program. Social worker will participate in activities that obtain feedback from clients and work to enhance services as needed.

    Grade Determinations :

    GS-9 Social Worker

    Experience, Education and Licensure. GS-9 is the entry level grade for the GS-185 social work series and is used for social workers with less than one year of post-MSW experience and for social workers who are not yet licensed or certified at the independent practice level. Social workers at the GS-9 level are working toward completion of prerequisites for licensure or certification. In addition, the candidates must demonstrate the KSAs in subparagraph (b) below. NOTE: Social workers with more than one year of post-MSW experience may be appointed at an advanced step within the GS-9 grade commensurate with their years of experience.

    (b)

    Demonstrated Knowledge, Skills, and Abilities

    1. Ability to provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds. This requires knowledge of human development and behavior (physical and psychological), and the differential influences of the environment, society, and culture.

    2. Ability to work with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group, and family counseling skills. Work with more complex problems is done under close supervision. With guidance from the social work supervisor, ability to assess the psychosocial functioning and needs of patients and their family members, and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills, and assistance needed.

    3. Basic knowledge of psychosocial treatment modalities and, under supervision, ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques.

    GS-11 Social Worker (full performance level) requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level.

    OR,

    In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. In addition, the candidate must demonstrate the KSAs in subparagraph (b) below.

This requires knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture.4. Knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures.

    The full performance level of this vacancy is 11. The actual grade at which an applicant may be selected for this vacancy is in the range of 9 to 11.

    Physical Requirements: Normal work of the position requires visual acuity (with the ability to read typewritten characters without strain, corrective lenses permitted), keen hearing, clear distinctive speech, and manual dexterity. This positionrequires potentially moderate levels of lifting and carrying (15-44 pounds). The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment.

    Work Environment: Work is performed primarily in an office setting; adequately lighted, temperature controlled, and ventilated. Office temperatures fluctuate periodically; involves everyday risks or discomforts which require normal safety precautions typical of such places as clinics, offices, meeting and training rooms or libraries; exposure to noise and dust on occasion. The work area is adequately lighted, heated, and ventilated. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures.
