JOB SUMMARY

The Coordination of Care Manager focus incorporates BHS and Care Management. These services are provided to ameliorate social and emotional stresses resulting from the interacting physical, social and psychological containments. The CCM integrates care management, social services, discharge planning, and pre- and post- hospital services to ensure clinical efficacy and best outcomes for our patients. The CCM, works to ensure the provision of quality health care along the continuum of care, decrease fragmentation, enhance the patient’s quality of life, efficiently use patient care resources, maximize cost containment opportunities, and improve successful post-hospitalization transition care. The CCM guides the integrated team in the functions of care coordination, facilitation of referrals, education, discharge planning and advocacy. The CCM manages the inpatient unit, Case Managers, UR and Crisis Response Team. The Coordination of Care Manager is also responsible for Integrated BHS.

PRIMARY JOB DUTIES

  • Serves as member of the Behavioral Health Services Leadership Team; attends weekly meetings with Director and other members of the leadership team.

  • Supervises/Oversees the staff and services provided on the inpatient unit and by the Case managers, UR and Crisis Response Team. Conducts monthly staff meetings and collaborates with the Director and Medical Director to attend monthly provider meetings.
  • Coordination across all BHS departments.
  • Works with the Director to develop staffing plans, operating budgets, and capital budgets and projects. Reviews, updates, and develops policies and procedures as needed to ensure compliance with state and federal regulations and DNV and CARF Standards.

  • Reviews candidate employment applications, conducts interviews, and makes hiring recommendations to the Director; coordinates with human resources.

  • Completes performance appraisals and employee disciplinary actions (in conjunction with Executive Director and CCMH Human Resources Department).
  • Maintains statistics as applicable and participate in QAPI activities. Have a strong commitment to code of ethics. Leads performance improvement activities with innovative thinking.
  • Has strong financial knowledge, budgets, manages productivity and develops new service offerings.
  • Ensures that all staff is competent to deliver appropriate patient/client care; works with the Director to identify and implement education and training for staff as needed.

  • Collaborates with physicians, nurses, case manager and other disciplines involved with care of the patient to foster a coordinated approach to patient treatment. Communicates with physician regarding the medical plan of care, anticipated discharge from hospital or treatment, and consideration of alternative setting. Facilitates and impacts process issues to avoid delays in patient treatment. Intervenes with appropriate individual/departments regarding delays in service that may have an impact on quality of care and/or length of stay. Provides feedback to the BHS Director regarding delays to constantly improve the process.

  • Function as a liaison to external agencies including other mental health centers, inpatient units/state facilities, rehab/residential facilities, assisted living/long term care facilities, public health and other community- based resources, as identified.

  • Demonstrates commitment to work partners to help each other reach mutual goals and learn from each other. Demonstrates actions and behaviors that consistently promotes trust, respect, positive attitude and promotes team morale

  • Available for peer and subordinate consultation regarding problematic cases or cases demonstrating deviations from the plan of care

  • Conducts self in professional manner, using Standards of Behavior as outlined by CCMH

  • Maintains professional relationships with other departments, external organizations, service providers, physicians, and families of patients

  • Must be free from governmental sanctions involving health care and/or financial practices

  • Complies with the hospitals Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures. Works within the scope of the Wyoming Social Work Practice Act

  • Performs other duties as assigned.

    JOB SPECIFICATIONS

  • Education: MSW, LCSW or RN
  • Licensure: As above
  • Certifications required: See Cardiopulmonary Resuscitation Certification Policy and Certifications/Education Requirements Policy.
  • Experience: Three years of experience in Leadership/Management preferred.

  • Essential Technical/Motor Skills: Ability to type
  • Interpersonal Skills: Excellent verbal communication skills; ability to mediate and interact with people from diverse populations.

  • Essential Physical Requirements: Sit, stand, and walk for extended period of time. Lift up to 10 pounds from floor to waist.

  • Essential Mental Abilities: Ability to define problems, collect data, establish facts and draw conclusions. Ability to apply concepts of fractions, percentages, ratios, proportions to practical situations.
  • Essential Sensory Requirements: Ability to hear and see

  • Exposure to Hazards: OSHA CATEGORY III. Identified job duties do not have any anticipation for contact with blood, contaminated body fluids or tissues. Employees working in healthcare facilities have the potential to be exposed to hazardous materials including: Hazardous Chemicals/Drugs, Waste Anesthetic Gases, Radiation, Latex, Biological Hazards, Respiratory Hazards and Ergonomic Hazards.
