PRIMARY CARE LIAISON
14 Step Salary Range $4,376 - $5,513 plus Generous Health/ Dental and PERS Benefits
Our Yakima based Administrative office has a full time M-F Primary Care Liaison position. Liaison will be responsible for facilitating primary care awareness of the aging services network, enhancing communication and referrals of older adults and caregivers from primary care clinics in our service delivery region. This position will be the key link between the NW GWEC and locally-based primary care providers, students in the medical profession, those in residency programs, patients, families, caregivers and social and health service workers. Experience Required: Bachelor Degree in Social Work, Human Services or related field, plus two years of experience providing direct human services in a social service agency. General computer knowledge required.
