Job Title: Client Care Supervisor, RN

Hours: Full Time

Reports to: Regional Manager

Location: Bellevue, WA

Compensation: DOE

Summary: Hiring a Client Care Supervisor, RN to assist clients in the Greater Eastside including, Bellevue, Redmond, Kirkland, Bothell, Woodinville, Issaquah, and Mercer Island. The Client Care Supervisor/RN is responsible for the clients throughout their lifecycle with the company. They take intake calls & provide information to potential clients, and do assessments, reassessments and supervisory visits. The CCS/RN ensures quality care and client satisfaction and is responsible for the maintenance of client and employee records. The CCS/RN will also provide oversight and supervision of nurse delegated and home health clients, and may be responsible to coordinate with per diem nursing staff. The CCS/RN participates in planning and accomplishing branch and company goals as directed by the Regional Manager and COO.

Duties and Responsibilities:

Be the first point of contact for our clients (current and potential), industry partners, and care providers, while providing excellent customer service.

Take phone inquiries from potential clients; provide accurate information about services and fees, and gather relevant information for client care/plan.

Schedule and perform in-home assessments and re-assessments.

Write care plans and ensure staffing supervisors have relevant information to review with assigned caregiver(s) prior to the start of care.

Assess and admit new or current clients for nurse delegation or home health needs.

Complete caregiver observation visits and client supervisory visit according to policy and document in client and caregiver file.

Update care plan as needed, communicate changes to caregiver(s), route to client for review and signature, and document the same.

Ensure best matches between caregiver and clients and correct certifications for Nurse Delegation and Home Health.

Ensure compliance with the care plan and accurate documentation of all care plan tasks.

Actively participate in staff training and operational meetings.

Adhere to all Family Resource Home Care policies, procedures, and state and federal regulations.

Other duties (including after-hours call) as assigned. Position may also be modified to accommodate the specific needs of assigned branch.

Requirements

Minimum Qualifications:

Active Washington State RN License

2 years of experience in hospital, home health or other long-term care setting

Active CPR Certification

Working Conditions:

The Client Care Supervisor/RN will spend 75% of their time in an office environment indoors. This will require time both spent at a computer station for office work and standing. The additional 25% of their time will be in various client homes performing assessments or observations.

The usual business hours for this role will be Monday through Friday 8:30am-5pm, however this role will require some evening and weekends to meet job requirements.

This position will require travel as appropriate, approximately 20% of the time.

This position will have moderate supervision.

Physical Requirements:

Role may require frequent standing, walking, sitting, keyboarding, and driving. Occasional twisting, climbing stairs, stooping, bending, kneeling or squatting. Seldom work above shoulders, and operation of equipment.

Position will require the employee to talk and hear.

Lifting can range from frequent 10 lbs. or less to seldom lifting of greater than 50 lbs.

Direct Reports:

This position will be indirectly supervising the per diem nurses.
