General Position Description: The Person Centered Planner is responsible for creating the Individual Support Plan (ISP) using Person Centered Thinking/Planning and working with the individual and their chosen network of support. Plans are submitted to the Department of Behavioral Healthcare, Developmental Disabilities and Hospitals (BHDDH). Person Centered Planners are contracted with the individuals through the Self-Directed Program. Perspectives employees are not eligible to apply.
Purpose : The Self Directed Plan Writer is a contracted position that assist individuals who Self Direct their contracts from BHDDH, to conduct Person Centered Planning meetings, write an Individual Support Plan that is submitted to BHDDH and meet quarterly with the individual to check in on progress of established goals.
Accountable for: Meeting with an individual/family/caregivers to get to know the person and assess needs and wants for their upcoming Individual Support Planning Meeting. Schedule a time, place and date to facilitate a Person Centered Planning Session. Complete an ISP form, review with individual/family/caregivers and submit to agency. Complete paperwork to agency for compensation which is as follows:
Initial Plans compensation is $500.00
Ongoing Plans compensation is $350.00
Completed Career Development Plan compensation is $180.54
Desired Qualifications: ï§
Experience in facilitating Person Centered Planning meetings ï§
Experience in writing Person Centered Plans for people with intellectual or developmental disabilities ï§
Experience in submitting ISPs that meet BHDDH criteria ï§
Understanding the unique nature and strengths and gifts of each individual ï§
Understanding community resources, community organizations and other natural supports ï§
Facilitate community mapping to match community supports to the individual ï§
Develop plans that are person centered and provide sufficient detail about how the plan will be implemented
