PRN - Social Worker

Conemaugh Nason Medical Center is seeking a PRN - Social Worker. The PRN - Social Worker is responsible for coordinating the level of care appropriate to patient needs in collaboration with the Case Management Team. Performs discharge planning based on high-risk assessment screening. Works with patient/family/physician to execute discharge plan. Provides direct intervention and expertise to the care team related to psychosocial support for assigned patient care areas. Uses social work skills to provide psychosocial support to patients and links patients to appropriate community resources.
