Meals on Wheels Case Manager

Responsible for managing the Meals on Wheels program with the Director of Case Management and Meals on Wheels coordinator to ensure homebound clients are eligible and receiving the Meals on Wheels benefit. Our nonprofit organizationcontracts with both Montgomeryand Berks County Area Agency on Aging to execute and comply with county contractual Meals on Wheels requirments for over 150 eligible Meals on Wheels clients. Currently, the program is growing, and we are in need of a social worker who is able to work at our organization and also perform home visits to assess, execute, and follow-up with Meals on Wheels clients in their place of residence. The role requires detail, compassion, and administrative competencies for assessing, enrolling, documenting, and ensuring county, state and federal requirments. Our organization, Boyertown Area Multi-Service has nine MOWs routes, and approximately 150 MOW clients, and over 100 MOWs delivery volunteers. This is a full-time posittion: Mon-Fri. 8:30 am- 4:30 pm.A degree in Social Work is a requirment. EOE
