Develops implements and evaluates individualized patient education plans during admission, transfer and discharge of patients.Assesses patient’s status per unit and organization policies and standards of practice.Develops, implements, and evaluates patient’s plan of care, making changes in the plan of care appropriate to the patient/family needs per unit’s standards of practice including physical, psycho-social, cultural, growth and development dimensions.Assesses, plans, implements, and evaluates treatment based on age specific criteria and provides care appropriate to the age of the patients served (neonate/infant, toddler, adolescent, adult and geriatric).Verifies patient identity, operative procedure and site, reviews diagnostic reports, verifies allergies and MD orders.Physician is informed in a timely manner of significant deviations from norms.Documents nursing care completely and legibly on appropriate forms.Delivers care in a nonjudgmental, nondiscriminatory, sensitive, courteous, and culturally competent manner.
