Successful Elements:
Provide individualized basic skills assessment and identify employment goals based on career interest, ability and personal development considerations
Develop Individual Service Strategy based on youths strengths and goals
Build one-on-one working relationships with participants to assist in mitigating challenges to obtaining and retaining employment
Facilitate educational activities in individual and group settings to remediate basic skills and increase functional educational levels
Provide career readiness and life skills classes for youth
Coordinate onsite and offsite career awareness field trips with employers and educational facilities
Provide comprehensive case management and follow-up to ensure positive outcomes and goals are met as required by contract
Provide referrals to appropriate community service or social service agencies
Establish detailed documentation of all client records to maintain organized database and generate all project reports as needed
Manage a caseload of 50-75 participants with a 100% eligible enrollment rate
Provide regular updates to Program Manager
Other duties as assigned
