The Heart & Vascular Special Procedure Coordinator will be responsible for providing nursing care across the continuum of care services related to patients that present with valvular disease and structural heart diseases that may require a broad spectrum of advanced surgical or interventional treatment and services. This position will be involved in research clinical trials, patient and professional educational programs and patient follow-up. The role of the Specialist Procedure Coordinator will evolve over time to meet the needs of the patients and of the advanced valvular disease and structural heart disease. These functions will be performed in accordance with the approved standardized procedures. This position will be responsible for the implementation of patient risk stratification, shared decision making, patient centered clinical care pathways tailored to desired patient outcomes and will have a main emphasis on the team approach.Minimum Qualifications:Bachelors degree in Nursing, NYS licensed/registered or eligible RN and a minimum of three years of acute care cardiac experience required. Must have demonstrated experience with coordinating clinical pathways for Valvular disease and structural heart disease patients. CPR-BLS successfully completed before hire.
