Main Function :

Under supervision, provide necessary supports to eligible persons that promote success at the highest level of independence possible.

Duties :

1. Professional Standards

Understand and adhere to all regulations, policies and procedures, including, but not limited to, Corporate Compliance, Personnel Policies, HIPPA, and Code of Conduct for Custodians of People with Special Needs

Respect the privacy of individuals and do not disclose confidential information to any unauthorized person

Demonstrate flexibility, politeness and respect in all interactions with individuals, families, and coworkers

Actively participate in training and ongoing professional development

Meet attendance and punctuality expectations

2. Person Centeredness

Develop supportive relationships with individuals and get to know their unique capabilities, personality and potential

Respect the dignity, preferences, and individuality of all persons

Maintain a person centered approach and support individual preference and choice to the extent possible under program guidelines

3. Teamwork

Support individuals served by providing assistance with daily living skills and oversight as needed

Assist Program Director in developing a variety of housing options that may provide safe and affordable permanent housing to program participants

Maintain ongoing relationship with community residence staff to support planning with individuals interested in exploring housing options

Possess a broad knowledge of community services which can be made available to participants, family members, service providers, and other significant support people

Communicate effectively with relevant Agency residential, outreach, and clerical staff to maximize support available to program participants

Maintain appropriate service, statistical, and financial records as required for coordination of services with other team members in a manner which respects confidentiality

Demonstrate professional accountability through respect, open communication, and active participation in a team environment

4. Accountability

Assist in the referral processes for Agency and community programs. Ensure responses to referrals are conducted in a timely manner.

Coordinate services with such programs to identify participants’ areas of need and delegate supports appropriately

Complete documentation based on program requirements following established guidelines and timeframes

5. Other Duties as Requested

Scope of Responsibility :

In addition to program-specific functions, responsibilities will include but not be limited to : assist participants with establishing and maintaining both mental and physical health care, promote independence in medication self-administration, provide situational counseling, benefits and entitlement counseling, engage in crisis intervention, facilitate the development of natural supports, and encourage self-advocacy.

All services will be provided in collaboration with program participants, utilizing a person-centered approach. Supports will be provided at the degree to which participant’s desire.

Education & Experience :

Bachelor’s Degree preferred. Associate's Degree plus three years of human service experience accepted, or equivalent combination of relevant education and experience.

Must have computer experience for documentation of services.

Conditions of Employment :

Must have a valid NYS driver license in accordance with the Agency Vehicle and Driving Policy and transportation necessary to meet program needs.

Schedule flexibility to accommodate participant needs, which will include evenings and weekends. Necessary to be able to hear clearly and possess good verbal communication skills.

Must be physically capable of performing job functions and must work well independently. May need to be outside in extremely cold, hot, or wet weather.

Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the New York State Office of Mental Health.

Must participate in successful database checks through the HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General and Specially Designated Nationals.
