Join a great non-profit group providing care and services to patients in the Rochester Community for over 46 years! This is a temp to hire community health role in a specialized healthcare setting.

The ideal candidate is a dedicated LPN who can provide one-on-one health care coordination and support to adults residing within a community-based settings. In this role, you will be working closely with the RN Project Leader and Lead LPN in managing all aspects of the patients healthcare.

Days/Hours: Monday “ Friday: 8:30 AM “ 4:30 PM. 37 ½ hour work week.

Salary: $20.17/hour. Excellent benefits available upon hire including a generous PTO package, Health Insurance, and more! Mileage reimbursement included.

Minimum qualifications for this position:

LPN license is required.

Spanish Bilingual desired.

1 year of experience with geriatric populations preferred.

Reliable transportation and valid drivers license.
