This program is dedicated to the improvement of quality of life of cancer survivors through education about the treatment they received and potential late effects through a multi-pronged approach that is directed by the program leaders and executed by all clinical care providers at the Wilmot Cancer Institute. Specifics of the role include: Leading quality /safety/research initiatives to meet the goals and metrics of the survivorship program. Attending and leading WCI committees/community programs associated with survivorship. In collaboration with the program director, development and implementation of strategies to further grow the survivorship program. Coordination and development of treatment summaries and survivorship care plans. Management of continuity of care for cancer survivors and their families/significant others, in collaboration with their respective teams. Collaboration with care providers to ensure provision of survivorship transition visits across all clinical Wilmot Cancer Institute divisions. MSN required, certification preferred. A minimum of 3-5 years’ experience in Oncology nursing practice is preferred. Ability to work independently and self-directed is necessary. QualificationsRequires a Master's Degree in Nursing or related field and an active New York State license in good standing. Applicants will have 5 years experience as an RN with one to three years of leadership experience; or equivalent combination of education and experience. The Clinical Nurse Specialist influences the quality and delivery of care in various roles including consultant, researcher, mentor, and practitioner.
