Under the supervision of the Home Care Supervisor, the LPN is responsible for providing direct physical nursing care to the patient. This individual is responsible for providing the highest quality of patient care in the home and ensuring patient safety at all times according to accepted nursing practice standards
Performs nursing care to participants in their home as delegated by the Case Manager
Performs Medication administration and prepours
Performs Wound care
Performs Other tasks delegated by the Case Manager
Observes for specific signs and symptoms and changes in clinical or functional status, and reports changes to the Case Manager
Demonstrates appropriate knowledge base when performing nursing care to participants
Provides for the emotional and physical comfort and safety of the participant and family/caregiver
recognizes and responds to emergent situations
NYS LPN license in good standing
Minimum of 1 year experience in an acute or long term setting as an LPN, a desire to work in a team setting
