This position is based in the downtown NY office at 9 Pine Street. Following training and evidence of being able to work independently (approx. 1-3 months), you will be able to work from home 2-3 days/week.4 positions:Foster Care Liaison Case Manager (2 openings) will be the direct Plan contact for care coordinators and service providers, and will be responsible for monitoring access for children in foster care. Work closely with Enrollment, BH UM, PH UM, Network, Quality and other teams within the plan to ensure member’s needs are met timely.Medically Fragile Liaison Case Manager (2 openings) who will be the direct Plan contact for members, their families, and service providers including Health Homes, and will be responsible for Case Managing these members. Work closely with BH UM, PH UM, Network, Quality and other teams within the plan to ensure member’s needs are met timely.Responsibilities for all positions include performing case management telephonically and/or by home visits within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs. Primary duties may include but are not limited to:

Coordinates with external resources including state agencies, foster care providers and others to meet identified needs of the members and collaborates with providers.

Uses appropriate screening criteria, knowledge and clinical judgment to assess member needs.

Conducts assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment.

Monitors and evaluates effectiveness of care plan and modifies plan as needed.

Supports member access to appropriate quality and cost effective care.

Computer skills: You will be working daily with MS Word, MS Outlook, MS Excel, Facets, Macess, Care Compass, SharePoint and a Fax queue. (You will receive training on Anthem specific applications, however, you must be comfortable working on a laptop with web-based applications.)

Minimum requirements include:

MS or MA in social work, counseling, or a related behavioral health field or a degree in nursing;

Current active unrestricted license in NY as an RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist to practice as a health professional within the scope of licensure in NY.

3 years clinical experience in social work, counseling with broad range of experience expertise and knowledge

Experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders preferred.

Comfortable using programs such as MS Word, Outlook and Excel. (Note: If you are not computer savvy, it will hinder your ability to be successful in this position.)

Additional Qualifications:Foster Care Case Manager

Broad range of experience expertise and knowledge of the child welfare system, foster care healthcare requirements and the unique complex needs (including trauma) of this population or any combination of education and experience which would provide an equivalent background.

Knowledge of NYS child serving systems required to effectively be responsible for coordination with OCFS, LDSS, and the VFCAs for all shared children in foster care.

Medically Fragile Case Manager

Broad range of experience expertise and knowledge of medically fragile population, Knowledge of State child serving systems, health homes, and specialty providers responsible for addressing the healthcare needs of medically needy children, including the medically needy LOC population or any combination of education and experience which would provide an equivalent background.

Knowledge of NYS child serving systems and Health Homes required.
