Req Number 0030GXJob DescriptionFacilitates patient admission, discharge, intra-hospital and inter-facility transfer processes through clinical triage. Ensures appropriate level of care is provided to patients from admission to discharge. Identifies and addresses barriers to safe and timely patient flow. Gathers clinical information, obtains resource authorizations, and coordinates transportation, as necessary, for direct admission and inter-facility transfer patients.Responsibilities Include:

Facilitates clinical triage and placement of elective and emergent admission requests, regardless of admission source. Ensures clinical hand-off communication to accepting nurse takes place in a timely and high-quality manner.

Identifies conflicts or issues related to patient flow. Collaborates with clinical team, ancillary and support services to resolve issues and achieve maximum efficiency and clinical outcomes.

Rounds, as necessary, in the clinical areas to reconcile census and bed placement issues.

Addresses and resolves issues arising from increased census and decreased bed availability.

Facilitates daily patient flow meetings with stakeholders from the Emergency Department, Perioperative Services, Nursing, Case Management, and Ancillary Departments, as necessary.

Address any conflicts or issues arising from dissatisfaction with bed assignments.

Ensures patient information and data entry in patient's chart is accurate, including but not limited to insurance information, clinical documentation, transportation requests and patient demographics.

Evaluates, processes, tracks, documents and coordinates patient transfers for the successful referral and transfer of patients to the facility. May conduct recorded three-way calls between referring physician and facility physician.

Performs related duties, as required.

Qualifications

Bachelor's Degree in Nursing, required.

Current license to practice as a Registered Professional Nurse in New York State.

Minimum of five (5) years nursing experience, including bed assignment and admissions process, required. Previous critical care, Emergency Department experience, preferred.

Advanced Cardiac Life Support (ACLS), preferred.

Demonstrated leadership with conflict resolution and problem solving skills, required. Excellent communication and organizational skills, required.

Proficiency in MS Office, MS Outlook, required.
