The Peer Navigator will provide day to day support, assist with case management, and play an active role in the Veteran program participant’s case as they pursue housing stability. The VPN will function as a role model for the peer and use personal experience to aid the Veteran client in various ways while he/she is moving toward housing permanency and long term sustainability. The position will promote accessibility to services, outreach to targeted populations, and assist with case management, provide advocacy, as well as wraparound supports and services within the VA and in the community.ESSENTIAL DUTIES:
Assist peers in verbalizing personal goals for housing stability
Maintain a strong knowledge of VA and community resources
Support peers wellness and recovery goals if applicable
Encourage professional, educational and personal growth when appropriate
Act as an advocate for Veterans
Attend community outreach activities in order to educate and recruit new program participants.
Accompany Veterans to various appointments at: DSS, VA, SSA, court, etc.
Type case notes on client interactions
Maintain confidentiality
Attend team meetings, supervision and trainings as needed
Travel across Nassau and Suffolk Counties to meet with and assist Veterans as needed
Performs other duties related to the program as defined by the Regional Director
College degree or HS diploma with equivalent relevant experience
Must be a Veteran
Self-motivated and resourceful
Ability to use computers
Strong communication skills
Must be able to work as a team player or independently
Knowledge of local resources, Veteran entitlements or other benefits is a plus
Must have a valid NYS Driver’s License
