Description
We are a full service Home Health Agency focusing on Senior Care serving the entire Las Vegas area.
We are looking for an energetic & enthusiastic Full Time Registered Nurses (RN's) that can fulfill the clinical needs of our growing patient population. We are fully electronic & patient documentation is done via our Electronic Medical Records (EMR) system
Qualifications:
Graduate of a National League of Nursing accredited School of nursing
Current Nevada Registered Nurse License
Bachelor's degree and prior home health experience preferred and willing to travel
Job Duties:
1. Patient Care
1. Adheres to the Nursing Practice when providing patient care
2. Respects the patient's privacy and property
3. Uses correct body mechanics when providing care to prevent injury to self and patient
4. Conducts comprehensive physical assessments and obtains medical histories on patients in the home setting
5. Makes the initial evaluation visit
6. Regularly re-evaluates the patients nursing needs.
7. Initiates the plan of care and necessary revisions
8. Furnishes those services requiring substantial and specialized nursing skill
9. Initiates appropriate preventative and rehabilitative nursing procedures
Prepares clinical and progress notes, coordinates services, informs the physician and other personnel of changes in patients condition and needs
Counsels the patient and family in meeting and nursing related needs according to Plan of Care
Participates in in-service programs, and supervises and teaches other nursing personnel
Develops nursing diagnoses and plans of care according to individual patient assessments and health care needs
Uses effective communication techniques with patients and families
Identifies when patient goals are achieved; identifies and acts to resolve any barriers to goal achievement
Maintains high standards and provides high quality, culturally competent nursing care
Uses effective time management skills and conserves agency resources as appropriate
Identifies changes in the patient condition and informs the physician and other appropriate patient caregivers
Functions as a case manager in coordinating care between all disciplines
Contacts physicians to obtain new orders
Communicates clinical information to the Director of Patient Care Services/Nursing Supervisor at start of care and periodically throughout the certification period with changes in patient condition and discharge plans
Completes and submits documentation according to agency regulations
Attains and maintains proficiency in the following skills:
Infection Control and Wound Care
Infusion Therapy and Central Line care
Colostomy, Gastrostomy care and Enteral Feedings
Tracheostomy Care and Oxygen Therapy
Foley Catheter Care and Insertion
Glucometers
Management and understanding of complex disease processes
Supervision (RN's only)
1. Responsible for supervising and instructing Licensed Practical Nurses and Home Health Aides
2. Reports deficiencies to the Director of Patient Care Services
3. Assists with the orientation of new personnel

Requirements
Physical Requirements
1. Pass a physical examination after being offered employment and a health assessment annually thereafter.
2. Able to lift up to 50 pounds.
3. Requiring the exertion of 50 pounds of force occasionally and up to 20 pounds of force frequently, and up to 10 pounds of force constantly to move objects.
4. Perform one person transfer under routine and emergency situations, reposition a bedbound patient alone and able to sit, stand, bend stoop, kneel, pull, climb, reach, lift, and perform repetitive movements of the upper extremities.
5. Walk, lift, reach, stoop, stand, grasp, balance, climb, kneel and crouch.
6. Work in various inside and outside environments being subjected to weather changes, noise, vibrations, job hazards and atmospheric conditions.
7. Use visual acuity required for depth perception and color perception.
8. Visually inspect small defects and/or small parts.
9. Use a variety of measuring devices.
Assemble or fabricate parts at or within arm's length.
Observe general surroundings and activities.
Hear sufficiently to accurately perceive information at normally spoken word levels.
May be exposed to unsanitary conditions in some home settings.
May be exposed to high crime areas within the service community.
Endure exposure to weather and temperature extremes.
Use communication by means of spoken word to express or exchange ideas.
Safely work in situations of exposure to blood borne pathogens which may require specialized personal protective equipment.
Must be able to travel.
