Integrated Life Choices is seeking EFHPs to provide residential supports in the EFHPs home for individuals with developmental disabilities. It is common for individuals supported to have either behavioral and/or medical health needs.
What is the process?
Individuals with a developmental disability are matched with trained EFHPs whom share the same interests and hobbies as the person seeking supports. The size of the home, all who live in the home (family size/kids/animals) and the location of the home are all influences on if the individual and their family are interested in meeting with an EFHP candidate.
The individual supported and their guardian are offered meet and greets with all trained and appropriate EFHP candidates. The individual and their guardian/family support will then decide if they are interested in moving forward with the potential EFHP.
EFH Contractor Requirements/Abilities/Skills:
Valid Driver license, car insurance and current registration
Ability to pass a State/Federal and APS/CPS background check (all members in the home)
Must have two years of experience supporting an individual with developmental disabilities
Must have means of transportation (medical appointments, to and from day services/employment, preferred activities and personal needs shopping)
Computer/lap-top, printer, scanner and internet for daily documentations
Completion of all required trainings
Must have detailed verbal / written communication skills
Must understand and comply with all agency and state policy and procedures
Must be attend and participate in all meetings
Must maintain a clean and orderly home
