JOB DESCRIPTION “ JOB COACH

POSITION IDENTIFICATION:

Functional Title: Job Coach

Supervisor: Supported Employment Specialist (SES)/ Individual Living Specialist (ILS)

FLSA Status: Non Exempt

Grade(s): 4

POSTION SUMMARY:

The Job Coach provides assistance to consumers with developmental disabilities in understanding and successfully completing all required elements for their specific position of employment.

ESSENTIAL DUTIES & RESPONSIBILITIES:

1. Provide appropriate supervision to ensure the consumers well-being and that they achieve the necessary level of competence in their position of employment.

2. Follow the instructions outlined by the Supported Employment Specialist (SES), the consumer, and the Personal Support Plan (PSP) when applicable.

3. Record data as directed by the SES and outlined in program, turn in data sheets bi-weekly and maintain regular communication with FO-SES.

4. Train consumer how to travel to and from job site as necessary or provide transportation in the situations where this is funded.

5. Educate individuals on attendance, punctuality, and how to follow their schedule.

6. Assist individuals in understanding their specific job tasks and following all rules and regulations for their job.

7. Educate individuals on how to be more time efficient and produce higher quality results.

8. Teach and encourage professional employee to employee relationships and teach and encourage professional employee to supervisor relationships.

9. Ensure that all required documentation for the consumers job position is completed by the consumer as required by the employer, signed and accurate (timecard, timesheet, duties list, etc.).

10. Teach basic social skills, encourage positive interactions among family members, co-workers, and encourage community integration and socialization.

11. Communicate any incidents or events with the employer supervisor and SES. Complete an incident report when applicable.

12. Be willing and able to complete job if consumer is unable to do so, on a minimal basis.

13. Fulfill obligations as a mandatory reporter for abuse, neglect or exploitation.

14. Turn in personal timesheets to Family Outreach no later than noon Monday following the end of the scheduled pay period.

KNOWLEDGE, SKILLS, & ABILITIES:

Good communication, written, and verbal skills.

Be comfortable working with people of different age groups and from different backgrounds, possibly with disabilities of varying levels.

Ability to handle any situations which arise on the job with employer, customers and client, provide satisfactory assistance and set a good example of appropriate social behavior.

Must possess ability to understand programs designed for consumer and collect concise data.

MINIMUM QUALIFICATIONS:

Minimum age of 18.

High School diploma, minimum required.

Experience working with young adults/adults with Developmental Disabilities.

Prior experience in job development or Day Habilitation setting preferred.

Ability to identify emergency situations and take appropriate actions. CPR & First Aid Training or ability to

complete this training with FO.

Requires a valid Montana Drivers license, a reliable vehicle, an acceptable driving record (no convictions for DUI or reckless driving within the last three years, and no more than two moving violations within the past three years) and successful completion of a background and criminal history check.

Knowledge of Developmental Disabilities, ability to work independently, ability to request assistance when needed to further the well-being of assigned individuals.
