We are searching for Case Managers and/or individuals who can help with currently unsheltered and chronically homeless into stable housing within Hennepin, Ramsey, Blue Earth, Washington, and Olmsted Counties.
Responsibilities include:
Developing and maintaining up to a 15-client caseload
Providing referral to community resources for mental health, chemical health, physical health, housing, benefits, and employment
Supporting participants in the development and achievement of client-centered goals
Enhancing client's ability to manage their mental health symptoms
Developing close working relationships with internal and external referral resources
Schedule will be: negotiated
Required qualifications:
Current and valid driver's license with current and valid auto insurance and a clean driving record
Experience interacting with or working with people who experience homelessness, mental illness, and/or chemical dependency or enrolled
OR
Enrolled in a college level field of study
