Title:Social WorkerID:7741Department:Social WorkFacility Name:Royal Braintree Nursing and Rehabilitation CenterLocation:MA - BraintreeRegion:South ShoreDescriptionSocial WorkerNursing Home/Rehab Center – Braintree, MA(Note: This is a Fulltime position)This premiere Skilled Nursing Facility is looking to add a Social Worker to their team.The Social Worker is expected to have working knowledge of state regulations set forth by the Department of Public Health and a basic understanding of state, federal and managed care insurances.Duties and Responsibilities

Upon admission, implement admission assessments, care plans and determine discharge plan.

Participate in 48 hour care plan meetings, family meetings as needed and discharge planning meetings.

Complete Minimum Data Sets (MDS) interviews and assessments in timely manner and implement care plans according to results.

Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Executive Director/Administrator as requested/required.

Participate in weekly Medicare meetings, work closely with rehab and case management to develop discharge plan for short term resident to ensure success back to community.

Provide all appropriate community referrals for short term residents prior to discharge to ensure safe discharge back to community.

Remain available and provide consultation to residents/families in efforts to solve their questions or concerns.

Collaborate with in-house Hospice providers on referrals and maintain communication on residents on hospice services. As well as attend monthly hospice CCM meetings.

Maintain Quarterly, Annual, Interim, Room Change, Hospitalization and Change of Status notes in resident’s medical record, as well as updating care plans accordingly.

Participate and attend weekly care plan meetings with Unit Manager; complete care plan summary of meeting following care plan meeting.

Maintain a quality working relationship with outside health care facilities and community organizations.

Work closely with our legal team to ensure Guardianships, Conservatorship and Roger’s Authority’s remain up-to-date. Facilitate and ensure legal paperwork is completed by physicians and returned to legal team in a timely fashion.

Maintain a positive, productive working relationship with all other departments within the building in order to promote team atmosphere.

Requirements

Social Work License (LSW) in Massachusetts, LCSW preferred

Social Work experience - 1 year minimum

Long Term Care experience - 1 year preferred

Short Term Rehab/Discharge Planning experience – 1 year preferred

Self-motivated with strong organizational skills and superior attention to detail

Ability to multi-task in fast pace work environment

Possess the skill to work independently and also be an important member of interdisciplinary team.
