DON: Nursing Care. Provides skilled nursing care when needed and performs specific treatments and administers medications as required or ordered in the Medical/Service Plan and Nursing Care Plan.
Observes and trains staff to aseptic techniques as required.
Trainer. Educates clients/patients, family, or representatives regarding the patient’s illness, procedures, treatments, including self-care, prevention, and health maintenance.
LPN: Must be physically able to lift, bend and stretch, climb stairs; Negotiate difficult pathways in and about the patient’s home.
Support client/patient in turning, standing, sitting, transferring, and walking activities.

Providing routine daily nursing care services, assisting under the supervision of the Registered Nurse (RN) reports to Director of Nursing (DON) or RN.
GNA:. Typical duties of Geriatric Nursing Assistants include dressing, bathing and feeding patients, moving and repositioning bedridden patients, changing bed linens, taking temperature, pulse and blood pressure, preparing and transporting patients and recording care. Transfers and positioning, grooming, assisting with meals, maintaining accurate documentation and other duties that may be assigned by the RN.
Performs personal care activities, including: Bathing, Shampooing, Skin care/nail care, Oral hygiene, Shaving, Dressing and other duties as needed and assigned
RN: Support client/patient in turning, standing, sitting, transferring, and walking activities.

PT(Physical Therapist): Consulting with patients to learn about their physical condition and symptoms
Diagnosing movement dysfunction and developing a treatment plan
Teaching patients how to properly use therapeutic exercise techniques
Providing stimulation or massage to promote healing
Assisting patients with the use of equipment such as wheelchairs or walkers
Maintaining patient records and keeping track of goals and progress
Advising the patient and family about in-home treatment options and exercises
RT( Respiratory Therapist): Interview and examine patients with breathing or cardiopulmonary disorders
Consult with physicians to develop patient treatment plans
Perform diagnostic tests, such as measuring lung capacity
Treat patients by using a variety of methods, including chest physiotherapy and aerosol medications
Monitor and record patients’ progress
Teach patients how to take medications and use equipment, such as ventilators
SOCIAL WORKER: Diagnose and treat mental, behavioral, and emotional disorders, including anxiety and depression
Provide individual, group, family, and couples therapy
Assess clients’ histories, backgrounds, and situations to understand their needs, as well as their strengths and weaknesses
Develop a treatment plan with the client, doctors, and other healthcare professionals
Encourage clients to discuss their emotions and experiences to develop a better understanding of themselves and their relationships
