Position Summary: The Emergency Department Care Coordinator works
in conjunction with the ED providers and admitting physicians to
determine the appropriate admission status and level of care. Utilizing
defined utilization management criteria, the ED Care Coordinator
provides clear guidance regarding status and level of care for all
patients being admitted to the hospital or referred to the Observation
Unit. All available provider documentation and test results are
reviewed to ensure all required supportive documentation is present to
justify status and level of care prior to patient leaving the ED. The
ED Care Coordinator is also responsible for direct admission screening
and room assignment, and is the facilitator for provider to provider
communication regarding the admission status.The ED Care Coordinator communicates throughout the shift with
Admitting, House Supervisor, PACU, SDS, and nursing units to assure that
all pre-scheduled surgeries/procedures remain at the pre-approved status
as they move through the various stages of treatment and recovery. If
the patient's clinical status dictates, the ED Care Coordinator will
revise the status and facilitate provision of the correct level of care.
Additionally, the ED Care Coordinator is responsible for facilitating
patient flow from the ED to the inpatient and observation units by
appropriately assigning patient rooms in a timely manner.The ED Care Coordinator is responsible for assessing, identifying and
discussing with patients/caregivers, safe discharge plans when admission
or observation status is not indicated. Once a safe and appropriate
discharge plan has been agreed upon by all involved parties, the ED Care
Coordinator is responsible for coordination and communication of the
plan.Education: Associates degree in Nursing required. Bachelor of
Science in Nursing or related field preferred.Licensure/Certification: Registered Nurse license from the State of
MD required. Certification in Case Management/Utilization Management
preferred. BLS - Basic Life Support certification is required, but will
provide training if necessary.Experience: Minimum of four years of nursing experience in acute
care setting. Previous experience in utilization review and discharge
planning desired.Skills: Strong computer skills with experience using Microsoft Word,
Excel, and Outlook desired. Must demonstrate strong critical thinking
skills as demonstrated by an ability to correctly apply utilization
criteria, identify and coordinate appropriate discharge planning options
for routine and non-routine patient situations. Demonstrated excellent
communication skills.MAYBE REQUIRED TO WORK ADDITIONAL SHIFTS OR VARYING HOURS TO MEET THE
OPERATIONAL NEEDS OF THE DEPARTMENT.
