Care Coordinator-Intensive Care Management

The Care Coordinator-Intensive Care Management is responsible to respond to the needs of adults receiving Habilitation Services or Children on the Child Mental Health Waiver. One year experience working with adults and/or children with mental illness. Valid driver’s license. Write, reads, comprehends and speaks fluent English. Use of usual and customary equipment used to perform essential functions of the position.Preferred: Two to four years’ experience working with adults and/or children with mental illness. Knowledge of Habilitation services and Child Mental Health Waiver services.
