Job Description

We are seeking a full-time Registered Nurse (RN) Case Manager for our Boise, Idaho branch! The RN Case Manager is responsible for the overall coordination of client care for all disciplines. Case managers are to see that the client is provided with the professional and paraprofessional levels of care appropriate to the hospice client and family needs.

    ESSENTIAL FUNCTIONS

    • Perform or participate in admission process according to policy and procedures.
    • Complete Initial Assessment for clients within 48 hours of hospice admission (unless directed by Physician, DON, or family requests otherwise).
    • Complete comprehensive assessments no later than 5 days of hospice admission.
    • Assess the support system in the home and educate accordingly.
    • Responsible for creating client-specific plan of care/treatment in coordination with Medical Director and Interdisciplinary Team.
    • Return completed clinical documentation to the office per policy and procedure.
    • Complete supervisory visits according to time guidelines when a hospice aide/homemaker is ordered.
    • Document nursing interventions according to nursing plan of care on subsequent visits.
    • Complete re-certifications at appropriate time intervals as defined by Conditions of Participation (COPs).
    • Complete written verbal orders for any changes in medications or treatments or changes in level of care.
    • Update medication record as needed to ensure accuracy prior to each IDT meeting.
    • Update and rewrite the Aide Plan of Care (POC) with every certification period or when there is any vital change in condition to the POC, as necessary.
    • Participate in the IDT meeting to review each client every two weeks. This includes a summary of problems, interventions, goals, changes in condition or level of care, and changes in visit frequency for each discipline.
    • Communicate and coordinate care with all members of the IDT as client/family condition warrants.
    • Notify Medical Director, team leadership, attending physician, pharmacy, DME, and other team members as appropriate when a client expires or goes off service.
    • Provide post-mortem care as appropriate.
    • Complete discharge and other client status forms, and dispose of controlled substances/ drugs according to policy and procedure.

    ADDITIONAL EXAMPLES OF WORK PERFORMED

    • Accompany hospice aides for visits as needed.
    • Respond to staff, patient, and family needs, which may be after hours or on weekends.
    • Participate in on-call rotation for after hours and weekends.
    • Performs related duties as assigned or required.

    Qualifications

    JOB SPECIFICATIONS

    • Registered Nurse (RN) with a current license to practice in Idaho.
    • Minimum of one (1) years nursing experience.
    • Current CPR certification.
    • Schedule flexibility and travel required.
    • Ability to communicate effectively verbally and in writing.
    • Must have valid driver’s license, vehicle insurance, and reliable transportation.

    WORK ENVIRONMENT AND PHYSICAL DEMANDS

    • Duties are primarily performed in patient residence or an office environment and require sufficient personal mobility and physical capability to permit employee to function in these environments.
    • Must be able to lift up to twenty (20) pounds.
    • Required to work irregular hours including evening and weekend availability.
    • Travel to client residences and other locations required.
