We are looking for a Hospice Patient Care Supervisor to support St. Joseph Hospice, Sonoma County.Location: Sonoma County, CaliforniaSchedule: Full time - 64 Biweekly hoursShift: 8 Hour - DaysJob Summary:Provides oversight for the assessment, admission and delivery of nursing and/or home health aide services to hospice patient/family members. Supervises the clinical practice of the nursing and/or home health aide staff. Understands the relationship between nursing practice and the reimbursement system monitoring productivity standards. Provides support and encouragement to nursing and/or home health aide staff in working with dying patients and grieving families. Serves as a facilitator to the interdisciplinary team in maintaining the integrity of the patient/family Plan of Care.Skills:
Good verbal and written communication skills.
Sound understanding of pain and symptom management for dying patients.
Ability to provide team leadership
Minimum Position Qualifications:Education:
Registered Nurse
Experience:
2 years nursing experience in hospice, home health or oncology
Licenses and Certifications:
California Registered Nurse
CPR Certification
Preferred Position Qualifications:
Education:
Bachelor's Degree in Nursing
Coursework/Training - Health care management training
