This position is a regular, full-time, benefited position. We are open to exploring a flexible schedule or other part time hours while still meeting the needs of the program. The Nurse Practitioner (NP) is a part of our Redwood Coast PACE team and
Provides primary care to participants of Redwood Coast PACE.
Performs intake assessments.
Is an active participant of the Interdisciplinary Team and coordinates to develop and implement a therapeutic
Plans of Care.
Establishes collaborative relationships with other medical providers and specialists.
Qualified candidates will have graduated from an accredited nurse practitioner schooling program and five (5) years of nursing experience, with at least three (3) years of experience with a frail or elderly population.
