Medical Coordinator

The Medical Coordinator is responsible for assessing, facilitating and monitoring all components of client medical services. Additional responsibilities include ensuring compliance with regulations in policy development and implementation according to State Licensing and ORR, as well as on-going program refinement based on advances in the program.

Advance typing and computer skills required to perform the responsibilities.

Schedule and comply with required medical, dental and specialize appointments and timelines.

Obtain, compile, and maintain EMR, medical documentation on each admission health related information, and discharge. Ensure proper daily documentation in Southwest Key Program’s database ETO, PCC, PCU and ORR-UAC Portal and maintain hard copy of required documents and EMR available.

Respond, assets, and comply with client’s medical complaints, urgent and emergency medical needs immediately.

Maintain and review for accurate and complete list of medical supplies, prescribe medications, and over-the-counter OTC medications according to the standing orders.

Prepare youth for examinations and record the medical intake, take vital signs, height and weight. Initiate routine lab tests. Completed required medical assessments

Participate and advocate during any medical procedures performed by medical providers and health services.

Assist the medical providers’ staff with examination procedures when needed, such as patient screening /triage.

Dispense medication according to the supervision of self-administration of medication (SAOM) schedule, policies and procedures according to the SWK and Licensing standards.

Collect, maintain, and submit daily, weekly, monthly, quarterly, and annual reports to supervisor as required.

Facilitate and conduct supervision of SAOM practicum, universal precautions, TB 101, allergies and other medical related training to employees and clients according to the program needs.

Provide a fail-safe plan with medication count, label accuracy, and supervision of SAOM, including over-the-counter, prescribed and psychotropic medication.

Educate and counsel youth concerning their medical status, disease, treatment, and prevention of disease and other health related topics.

Schedule and provide transportation to clients on medical appointments with assistance from floor staff as needed.

Able to react to change productively and handle other essential tasks as assigned.

Develop and maintain effective communication and working relationships with staff, co-workers, physicians, and UM

Collaborate, develop and expand medical, dental and other related services (e.g. labs, X-rays) with medical providers.

Maintain Health Information Privacy HIPAA. Respond and comply to individual emergency medical needs immediately.

Maintain professional, confidential and ethical standards guidelines by Southwest Key and other entities.

High School Diploma/GED

AND

Certified Medical Assistant (CMA); Certified Nursing Assistant (CAN); Certified Medical Office (CMO); or related medical field certification (e.g. phlebotomy, pharmacy etc), WITH 1 (one) to 2 (two) years of experience in a health care setting with pediatric/adolescence population

OR

Associate/Bachelor degree in any related health - Minimum 1 year of full-time experience in a hospital, clinic, medical record department or physician’s office. (including any volunteer or internship experience)

Foreign Graduates in Medical Sciences (e.g. physicians and nurses): Applicants from countries or territories outside the USA must meet the above stated criteria.

Advance typing and computer skills.

Bilingual. (Spanish/English). Ability to read, analyze, and interpret common medical reports and legal documents.Cleared Tuberculosis test results.

Immunization documentation: Tetanus, diphtheria, pertussis (Td/Tdap), Varicella, Measles, mumps, rubella (MMR), Hepatitis A and B.

Cleared background check from appropriate entity.

Cleared drug test results (for Texas programs only).

Comply with annual training requirements/licensure/certifications.

Valid Drivers’ License.

Willingness to work a flexible schedule and be on-call evenings and weekends.

Must demonstrate exceptional:REASONING ABILITY: Communication skills, ability to communicate and respond to common inquiries or complaints from both clients and staff from a variety of disciplines, addressing complaints and resolving problems.Skills Preferred:

Cultural competence and sensitivity to the needs of a diverse Unaccompanied Minors population.

Experience must focus on community-based programs, and/or direct practical experience with UM

Bending, stooping, and lifting a minimum of 15-25 lbs required to complete daily tasks

Must have the ability to remain in standing position for extended periods of the work day

Ability to work a flexible work schedule is required dependent upon the department, facility or program in which s/he works and job duties assigned. Work schedules are subject to variation from time to time depending on the needs of the department, facility or program. The work is performed in a shelter setting, which operates 24 hours a day, 7 days a week, 365 days a year. In order to satisfactorily perform all job duties for this position, medical provider and staff are expected to be available to work at any time as directed by his/her supervisor.
