Case Manager for Prescott Area Shelter Services

Reporting to the Executive Director, the Case Manager will provide evidenced based case management services to PASS guests. The position is 30 hours per week; Sunday and Monday off, Tuesday 12-8 pm, Wednesday 12-8 pm, Thursday off, Friday 9 am- 5pm, Saturday 3 pm-8pm with two flex hours each week. The Case Manager must cover at least three evenings a week until 8 pm in order to be available when all of our guests are at the shelter.

Responsibilities:

Case Manager Job Duties:

Determines potential guests' admittance by completing phone-based intake interviews.

Admits new guests by reviewing records and applications; conducting orientations.

Completed all intake processes including determining need for medical and mental health treatment; reviewing doctor/therapist evaluations, treatment objectives, and plans.

Scheduled and manage regular case management meetings with guests.

Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.

Monitors cases by verifying guests’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support and handles emergent situations if they arise.

Maintain immediate documentation of guests' records by reviewing and adding to case notes; logging events and progress.

Communicates clients' progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and identifying treatment influences.

Provides weekly community shelter meetings to guests’, facilitates on campus educational opportunities such as tenant/landlord workshops, life skills classes including a variety of topics, one on one parenting mentorship, and financial readiness classes.

Prepares clients' exit date of shelter by reviewing and coordinating transition plans; providing community resources to aid in the transition.

Improves financial status by analyzing results; monitoring variances; identifying trends; recommending actions to management.

Supports guests’ ability to meet their budget by monitoring expenses and assisting with the implementation of cost-saving actions.

Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics.

Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; and balancing work requirements with learning opportunities.

Enhances organization’s reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

Attends Community Collective Impact Group and the Community Circle scheduled meetings.

Case Manager Skills and Qualifications:

The Case Manager will be thoroughly committed to PASS’ mission. Concrete demonstrable experience and other qualifications include:

Documentation Skills, Analyzing Information, Decision Making, Research Skills, Verbal Communication, Written Communication, People Management Skills, Conflict Resolution, Integrity, Basic Computer skills, Ability to connect with children and Team Player.

A Bachelor’s Degree preferred but may be substituted for a minimum of three years of experience working with homeless and/or underserved populations.

Ability to work with minimal supervision.

Valid driver's license and reliable transportation; meeting and shelter activities require driving.
